Public Engagement Events on Integrated Community Nursing and Therapies

Bridgewater Community Healthcare NHS Trust (Bridgewater) is working on a project with Wrightington, Wigan and Leigh NHS Foundation Trust (WWL) and Wigan Council to look at new ways of delivering a large group of health and care services in the Wigan Borough. This project known as Integrated Community Nursing and Therapies (ICNT) Transformation is  based on Wigan Borough Clinical Commissioning Group’s vision for much closer working between services and organisations for the benefit of patients.

The proposed new Integrated Community Nursing and Therapies (ICNT) service will coordinate out-of-hospital care services across the borough based on a personalised system that anticipates care requirements, with shared care planning based around the needs of the individual patient.  This means any individual will only have to tell their story once and will have one assessment, one key worker and one care plan - all supported by one integrated system. The project will also work  in partnership with Wigan GP Practices, 5 Boroughs Partnership NHS Foundation Trust (5BP) and Wigan Council.
We want to help patients and carers understand these proposals and work with you to develop the detail of how the proposed new service will be provided. To start this process we are running three patient engagement events across the Wigan borough on the following dates:

  • 30 September: 11.00am to 12.30pm: Meeting Room at Leigh Health Centre, The Avenue, Leigh.
  • 14 October: 9.30am to 12.00pm: Meeting Room, Worsley Mesnes Health Centre, Chandler House, Poolstock Lane, Wigan.
  • 29 October: 9.30am to 12.00pm: Meeting Room, Platt Bridge Health Centre, Rivington Avenue, Platt Bridge, Wigan.
    On arrival please report to the community reception in each of these venues. 

Our key objective is to provide the best possible service to our patients by listening to their needs, so please do come along to share your views with our programme team. Your feedback will help influence the design of future of health and care service provision across Wigan.
