New Integrated Discharge Hub Opened at Wigan Infirmary

A new Integrated Discharge Hub was officially opened at Wigan Infirmary on Wednesday 3 August in what has been a tremendously positive development for our Trust.

The Hub, based close to Trust Headquarters, houses an Integrated Discharge Team involving representatives from a whole host of organisations from across the Wigan Borough. The hub will host teams from; Wrightington, Wigan and Leigh NHS Foundation Trust, Wigan Clinical Commissioning Group (CCG), Bridgewater Community Healthcare NHS Trust, 5 Boroughs Partnership NHS Foundation Trust, Wigan Council and the Brick Project, a local homeless charity.

This project has been running for three months and is evidence of a strong partnership, with good relations, between the organisations involved, plus a unifying commitment to get the best possible results for patients in the Wigan Borough.

The issue that the new team has been attempting to resolve is to try and reduce the number of patients who have completed their medical treatment, and are ready to be discharged, in the Acute Trust. This, so far, has been managed with the Team producing some fantastic results. The number of patients in these circumstances has dropped from a daily average of 30-40 patients to 15-20 patients.

A large contributing factor to this success has been a new way of working, between teams from each organisation, which has been adopted. The teams now input information into a new electronic discharge tracker, which all services within the Integrated Discharge Team use. It has helped tremendously with communication and tracking of discharges. Also there is a joint approach to decision making around discharge process which has helped with patient discharge, leading to better communication with all members of different organisations.

Organisers of the project were thrilled to be able to officially open the Hub. Assistant Director   - Urgent Care and Integrated Discharge Team System Management, Chris Broadbent (CCG), said “This is a massive step in the right direction for discharge planning. By integrating the different organisations it helps to provide and deliver individual health and social care outcomes with safe and timely discharges. I am very proud of the team and what they have achieved in 3 months of working together as a fully-integrated discharge and collocated team.”

Among those in attendance from WWL were Pauline Law, WWL Director of Nursing, and Mary Fleming, WWL Director of Operations and Performance.

Pauline said, “I was delighted to take part in the official opening of Wigan Infirmary’s new Integrated Discharge Hub. Having seen the dedicated and productive hard work the team have managed to achieve so far, I have no doubt that this excellent collaboration with our fantastic partners will continue to go from strength to strength.”

Mary was equally happy. She said, “It was with great pleasure that I attended the official opening of the new Integrated Discharge Hub. Although they have only been together for a short time, the progress the team have made in their three months of operation has been astonishing. The team are a fine example of how strong collaboration with our superb partners in the Wigan Borough can bring about positive changes for our patients.”
